In All-on cases, extensive bone reduction is sometimes required. During suturing, excessive soft tissue may cover the healing caps. As a result, when delivering the immediate prosthesis, the soft tissue can become trapped, leading to incomplete seating or causing discomfort and pain for the patient.

Furthermore, excessive mobile soft tissue may lead to the resorption of what was originally abundant keratinized gingiva (KG), leaving only a minimal amount. This is quite unfortunate, and the issue is even more pronounced in the mandible.

Here, I would like to share a technique for reference, which I call the “mucosal anchorage suture.” By using the bony margin as an anchorage point, the mucosa can be properly positioned, preventing it from covering the healing caps.

This approach facilitates prosthesis delivery and helps preserve keratinized gingiva effectively.
