【Pterygoid implants vs Zygomatic implants】

▋ 患者狀況

患者是66歲女性，來的時候上顎已經拔除所有迷你植體。

下顎同樣為迷你植體與自然牙支持的 long span bridge。患者想全口重建上顎，下顎假牙目前還堪用 暫時不想重做。

▋ R’t maxillary sinusitis

術前 CT 可見，雙側上顎 zone 3 都嚴重缺少骨頭。右側可見有 OAC (oroantral communication) 以及 maxillary sinus cloudiness，臨床同時有 pus drainage 從右上牙齦。診斷為 maxillary sinusitis related to OAC。

治療計畫為先進行 OAC repair with buccal fat pad，同時進行 sinus cavity 灌洗。 術後兩週 Anbiotics with Curam。

▋ 術中發現

• 在 OAC repair 術後兩個月後，進行植牙手術。

• 在術中可發現，原來的 OAC 已經被 fat tissue seal 起來。

▋ CT evaluation after OAC repair and tx plan

術後復原完全，2M 後再確認 CT，可見 cloudiness 已經改善，但仍有些 mucosa thickening。患者已經完全沒有症狀。

治療計畫�

• Zone 1, 2 放置四支植體�。

• 左側 Zone 3 放置 zygomatic implant�。

• 右側 Zone 3 因為 OAC 的 defect, 所以往後放置 pterygoid implant。

▋ 小結

• 術後六個月的 CT 與臨床照片請參見貼文資訊圖卡第 7 頁。

• 可見右側 Pterygoid implant 與 左側 zygomatic implant 周圍軟組織保持健康的角化牙齦。

• 咬合面觀，可見在植牙分佈上，pterygoid implant 與 zygomatic implant 上的差異：pterygoid implant 出孔位會在 7 or 8 的位置，而且 buccal version。 zygomatic implant 出孔位在 5 or 6 位置，而且 palatal version。
