implant\u7684\u61c9\u7528\uff08\u4e0a\uff09\u3011<\/p>\n<p><\/p>\n<p>\u258b\u524d\u8a00<\/p>\n<p><\/p>\n<p>All-on-4 treatment concept \u61c9\u7528\u5728\u5168\u53e3\u5c11\u7259\u6216\u7121\u7259\u60a3\u8005\u5df2\u6578\u5341\u5e74\u6b77\u53f2\uff0c\u6210\u6548\u5ee3\u70ba\u4eba\u77e5\uff0c\u4e5f\u5927\u5e45\u63d0\u5347\u60a3\u8005\u8853\u5f8c\u6eff\u610f\u7a0b\u5ea6\u3002\u6839\u64da\u570b\u5916\u6587\u737b 2017 \u5e74\u767c\u8868\u4e4b Systematic review \u7d71\u8a08\u7d50\u679c\uff0c2\u5e74 All-on-4 \u690d\u9ad4\u5b58\u6d3b\u7387\u70ba 99.8%\uff0c\u7d71\u8a08\u4e4b\u690d\u9ad4\u5b58\u6d3b\u7387\u6700\u9577\u6709\u5230 10 \u5e74\u6642\u9593\uff0c\u7e3d\u8a08 24 \u7bc7\u671f\u520a\uff0c\u5171\u6536\u96c6 11743 \u690d\u9ad4\u6578\u91cf\uff0c\u690d\u9ad4\u5b58\u6d3b\u7387 94.8~100% \u4e0d\u7b49\u3002\u7531\u6b64\u53ef\u77e5\uff0cAll-on-4 treatment concept \u662f\u5177\u6709\u8db3\u5920\u8b49\u64da\u529b\u7684\u4e00\u7a2e\u624b\u8853\u8853\u5f0f\u3002<\/p>\n<p><\/p>\n<p>\u258b\u5c08\u70ba\u4e9e\u6d32\u4eba\u6253\u9020\u7684\u690d\u7259\u6d41\u7a0b<\/p>\n<p><\/p>\n<p>\u6839\u64da\u6bcf\u4eba\u81e8\u5e8a\u72c0\u6cc1\u53ca\u5404\u570b\u98a8\u571f\u540d\u60c5\u4e0d\u540c\uff0c\u674e\u91ab\u5e2b\u8a3a\u6240\u6709\u5225\u65bc Paulo Malo \u6559\u6388\u6240\u63d0\u51fa\u7684Malo Clinic protocol\uff0c\u5206\u985e\u8f03\u70ba\u7c21\u7565\uff0c\u56e0\u800c\u63d0\u51fa\u4e00\u5957\u5c6c\u65bc\u4e9e\u6d32\u4eba\u7a2e\u7684\u6d41\u7a0b\uff0c\u7a31\u4f5c Imperial clinic surgical protocol(Fig.1,2)\uff0c\u5176\u91cd\u9ede\u5728\u65bc\u6e1b\u5c11 cantilever \u9020\u6210\u7684\u690d\u9ad4\u5931\u6557\uff0c\u4e5f\u6e1b\u5c11\u4f7f\u7528\u8f03\u4fb5\u5165\u6027\u624b\u8853\u7684\u4f5c\u6cd5\uff0c\u5176\u7cbe\u795e\u5728\u65bc\u4ee5\u6700\u5c11\u7684\u624b\u8853\u50b7\u5bb3\u9054\u6210\u5047\u7259\u7acb\u5373\u4e58\u8f09\u53ca\u9577\u671f\u7a69\u5b9a\u6027\u3002\u5982\u679c\u7b2c\u4e00\u5927\u81fc\u9f52\u5340\u9aa8\u8cea\u8db3\u5920\uff0c\u4f9d\u7136\u53ef\u4ee5\u50b3\u7d71\u505a\u6cd5\u6253\u516d\u652f\u76f4\u7acb\u5f0f\u690d\u9ad4 (Axial implant)\u3002<\/p>\n<p><\/p>\n<p>\u258b\u5f8c\u7259\u9700\u8981\u6709\u81f3\u5c11 4mm crestal bone height<\/p>\n<p><\/p>\n<p>\u4f7f\u7528 tilted implant \u9664\u4e86\u589e\u52a0 A-P spread \u4e26\u6e1b\u5c11 posterior cantilever \u5916\uff0c\u56e0\u4f7f\u7528\u8f03\u9577\u7684\u690d\u9ad4\u80fd\u8207\u8f03\u591a\u76ae\u8cea\u9aa8\u63a5\u89f8\uff0c\u800c\u80fd\u627f\u53d7\u8f03\u5927\u7684\u54ac\u5408\u529b\u91cf\u3002\u9664\u4e86\u61c9\u7528\u5728 All-on-4 treatment concept \u5916\uff0c\u5728\u90e8\u5206\u5f8c\u7259\u591a\u9846\u7f3a\u7259\u4e5f\u53ef\u5584\u7528\u6b64\u9805\u6280\u8853\u7701\u53bb\u9f3b\u7ac7\u88dc\u9aa8\u7684\u8f9b\u82e6\u3002\u6839\u64daQueridinha BM. 2016 \u5e74\u6587\u737b\u4e94\u5e74\u7d71\u8a08\u4e5f\u6709 96.7% \u6210\u529f\u7387\u3002<\/p>\n<p><\/p>\n<p>Enrico L. Agliardi \u65bc 2015 \u5e74\u63d0\u51fa\u90e8\u5206\u5f8c\u7259\u7f3a\u7259 Trans-sinus tilted implant \u5206\u985e (fig 3) \uff0c\u5efa\u8b70 tilted implant \u9700\u8981\u6709\u81f3\u5c11 4mm crestal bone height\uff0c\u53ca 4mm apical bone \u624d\u80fd\u8b93 Trans-sinus tilted implant \u8db3\u5920\u7a69\u5b9a\uff0c\u674e\u91ab\u5e2b\u4e5f\u662f\u5efa\u8b70\u81f3\u5c11 4mm crestal bone height \u6210\u529f\u7387\u6703\u4e0a\u5347\u8a31\u591a \u3002\u7136\u800c\uff0c\u65bc\u5168\u53e3\u690d\u7259\u60a3\u8005\uff0c\u5f8c\u7259\u5340\u9aa8\u91cf\u66f4\u5c11\u6642\uff0c\u9808\u6e96\u5099\u9874\u9aa8\u690d\u9ad4 (zygoma implant )\u7684\u4ecb\u5165\u3002\u82e5\u60a3\u8005\u5c0d\u9874\u9aa8\u690d\u9ad4\u63a5\u53d7\u5ea6\u4e0d\u9ad8\uff0c\u53ef\u6539\u4f7f\u7528 Trans-sinus tilted implant \u6216 pterygoid implant \u4ee5\u50b7\u5bb3\u8f03\u5c0f\u65b9\u5f0f\u9054\u5230\u76f8\u540c\u6548\u679c\u3002<\/p>\n<p><\/p>\n<p>\u258bTrans-sinus tilted implant\u61c9\u7528<\/p>\n<p><\/p>\n<p>Trans-sinus tilted implant(fig.4,5) \u76f8\u8f03\u539f\u672c Tilted implant \u4f5c\u6cd5\u66f4\u70ba technique sensitive\u3002\u5728\u624b\u8853\u904e\u7a0b\u4e2d\u8981\u6210\u529f\u9054\u5230\u8db3\u5920primary stability\u901a\u5e38\u53ea\u6709\u4e00\u5230\u4e8c\u6b21\u6a5f\u6703\uff0c\u64cd\u4f5c\u8005\u5fc5\u9808\u8981\u6709\u8db3\u5920\u7684\u7d93\u9a57\u624d\u6703\u6210\u529f\u3002<\/p>\n<p><\/p>\n<p>\u6839\u64da 2013 \u5e74 Paulo Malo \u767c\u8868\u7684\u6587\u737b\uff0c\u65bc 70 \u540d\u60a3\u8005\u65bd\u884c All-on-4 treatment concept with immediate loading\uff0c\u5171 83 \u652f trans-sinus tilted implant\uff0c\u5176\u4e09\u5e74\u5b58\u6d3b\u7387\u70ba96.3%\u3002\u5728 Marginal bone resorption \u90e8\u5206\uff0cstraight implant , conventional tilted implants \u4ee5\u53ca tans-sinus tilted implants \u4e4b\u9593\u5247\u6c92\u6709\u986f\u8457\u5dee\u7570\u3002<\/p>\n<p><\/p>\n<p>\u65bd\u884c Trans-sinus tilted implant \u6280\u8853\u6642\uff0c\u4f7f\u7528\u5c0e\u5f15\u5f0f\u690d\u7259\u624b\u8853 (Guided surgery) \u53ef\u4ee5\u6e1b\u5c11\u4eba\u70ba\u5931\u8aa4\uff0c\u4f46\u5176\u7f3a\u9ede\u662f\u690d\u5165\u690d\u9ad4\u6642\u624b\u611f\u6703\u6bd4\u8f03\u5dee\uff0c\u8f03\u4e0d\u6613\u5bdf\u89ba\u8aa4\u5dee\u6539\u8b8a\u690d\u5165\u7b56\u7565\u3002\u540c\u6642\uff0c\u4e5f\u6703\u56e0\u70ba anterior sinus wall \u7684\u963b\u529b\uff0c\u690d\u9ad4\u8f03\u6613\u6ed1\u5165\u9f3b\u7ac7\u8154\u55aa\u5931\u690d\u9ad4\u521d\u7d1a\u7a69\u5b9a\u5ea6\u3002\u56e0\u6b64\u674e\u91ab\u5e2b\u8a8d\u70ba\u4f7f\u7528\u5c0e\u5f15\u5f0f\u690d\u7259\u624b\u8853\u4e4b\u524d\uff0c\u5fc5\u9808\u6709free hand\u624b\u8853\u6280\u5de7\u70ba\u57fa\u790e\uff0c\u4ee5\u4fbf\u7acb\u5373\u6539\u8b8a\u690d\u5165\u7b56\u7565\u3002<\/p>\n<p><\/p>\n<p>\u258b\u674e\u91ab\u5e2b free hand technique \u5c0f\u8a23\u7ac5<\/p>\n<p><\/p>\n<p>\u2022 \u624b\u8853\u6280\u5de7\u4e00\uff1a\u6253\u958b\u5074\u7a97\uff0c\u7ffb\u958b\u9f3b\u7ac7\u7c98\u819c\uff0c\u5229\u7528\u62d4\u7259\u7aa9\u8ef8\u5411\u6216\u5c0e\u5f15\u624b\u8853\u627e\u5c0b\u6b63\u78ba\u50be\u659c\u89d2\u5ea6\u3002<\/p>\n<p><\/p>\n<p>\u2022 \u624b\u8853\u6280\u5de7\u4e8c\uff1a\u4ee5\u89aa\u773c\u76f4\u8996\u65b9\u5f0f\uff0c\u78ba\u8a8d\u690d\u9ad4\u80fd\u9806\u8457\u5099\u5b54\u65b9\u5411\u524d\u9032\uff0c\u907f\u514d\u6ed1\u5165\u9f3b\u7ac7\u8154\u3002<\/p>\n<p><\/p>\n<p>\u2022 \u624b\u8853\u6280\u5de7\u4e09\uff1a\u4e86\u89e3\u690d\u9ad4\u7279\u6027\uff0c\u6839\u64da\u690d\u9ad4\u6839\u5c16\u76f4\u5f91\u5099\u5b54<\/p>\n<p><\/p>\n<p>\u2022 \u624b\u8853\u6280\u5de7\u56db\uff1a\u9f3b\u7ac7\u8154\u56de\u586b\u9aa8\u7c89\uff0c\u842c\u4e00\u672a\u4f86\u690d\u9ad4\u9700\u8981\u91cd\u690d\uff0c\u53ef\u7528\u7c97\u7684\u690d\u9ad4\u7f6e\u5165\u5373\u53ef\u3002<\/p>\n<p><\/p>\n<p>\u2022 \u624b\u8853\u6280\u5de7\u4e94\uff1a\u8981\u9810\u5099 zygoma implant \u6216 pterygoid implant\u6551\u63f4\uff0c\u5fc5\u8981\u6642\u4e09\u96bb\u690d\u9ad4\u5148\u53d7\u529b\uff0c\u7b49\u5f85\u690d\u9ad4\u9aa8\u6574\u5408\u5f8c\u63a5\u51fa\u3002<\/p>\n<p><\/p>\n<p>\u5982\u679c\u6709\u4e9b\u60a3\u8005\u4e0d\u9069\u5408\u505a Trans-sinus tilted implant\uff0c\u6216\u662f\u8a55\u4f30\u5f8c\u6709\u8f03\u9ad8\u7684\u98a8\u96aa\u8853\u4e2d\u55aa\u5931\u521d\u7d1a\u7a69\u5b9a\u5ea6\uff0c\u8981\u9810\u5099 zygoma implant \u6216 pterygoid implant \u7576\u505a\u89e3\u6551\u65b9\u6848\u3002<\/p>\n<p><\/p>\n<p>\u6216\u5176\u9918\u4e09\u96bb\u690d\u9ad4\u7a69\u5b9a\uff0c\u4e5f\u53ef\u5148\u7528\u4e09\u96bb\u690d\u9ad4\u652f\u6490\u5047\u7259\uff0c\u5728\u60a3\u8005\u54ac\u5408\u529b\u8f03\u5f31\u7684\u60c5\u6cc1\u4e0b\u4ecd\u6709\u6a5f\u6703\u53ef\u4ee5\u7acb\u5373\u4e58\u8f09\uff0c\u552f\u7368\u53ea\u9760\u4e09\u96bb\u690d\u9ad4\u652f\u6490\u7684\u5047\u7259\u8f03\u6613\u65b7\u88c2\uff0c\u54ac\u5408\u529b\u904e\u5ea6\u96c6\u4e2d\u690d\u9ad4\u5931\u6557\u7684\u98a8\u96aa\u6bd4\u8f03\u9ad8\uff0c\u60a3\u8005\u8853\u5f8c\u6eff\u610f\u5ea6\u4e5f\u6703\u56e0\u70ba\u55ae\u5074\u54ac\u5408\u8abf\u6574\u800c\u8b8a\u5dee\u3002<\/p>\n<p><\/p>\n<p>\u258bTrans-sinus tilted implant \u81e8\u5e8a\u61c9\u7528\u5be6\u4f8b<\/p>\n<p><\/p>\n<p>\u60a3\u8005\u5e74\u4e8b\u5df2\u5927\uff0c\u60a3\u6709\u4e73\u764c\u63a5\u53d7\u904e\u6cbb\u7642\u5df2\u7a69\u5b9a\uff0c\u7cd6\u5c3f\u75c5\u5df2\u63a7\u5236\u7a69\u5b9a\u3002Low smile line\uff0c\u9aa8\u8cea\u8f03\u9b06\u8edf\u3002\u900f\u904e tilted implant \u642d\u914d All-on-4 treatment concept \u65b9\u5f0f\u5b8c\u6210\u6cbb\u7642\u3002<\/p>\n<p><\/p>\n<p>\u258b\u690d\u9ad4\u7684\u9078\u64c7<\/p>\n<p><\/p>\n<p>\u690d\u9ad4\u7684\u9078\u64c7\u5728 Trans-sinus tilted implant \u61c9\u7528\u4e0a\u683c\u5916\u91cd\u8981\uff0c\u5fc5\u9808\u80fd bi-cortical fixation\u3002\u4e0b\u5217\u4ee5 Nobel Biocare \u7522\u54c1\u70ba\u4f8b\uff0cNobelSpeedy \u03a64mm \u690d\u9ad4\u6839\u5c16\u76f4\u5f91\u70ba 2.2mm\u3002 \u03a65mm\u690d\u9ad4\u6839\u5c16\u76f4\u5f91\u70ba 2.0mm\u3002<\/p>\n<p><\/p>\n<p>\u6545\u53ef\u4ee5\u85c9\u7531\u5099\u5b54 under preparation \u4f86\u9054\u5230\u521d\u7d1a\u7a69\u5b9a\u5ea6\u3002\u82e5\u662f\u4f7f\u7528 NobelReplace\u00ae Conical Connection \u690d\u9ad4\uff0c\u6839\u5c16\u70baround apex \u76f4\u5f91 2.56mm\uff0c\u5fc5\u9808\u8981\u64f4\u66f4\u5927\u6839\u5c16\u947d\u5b54\u624d\u6709\u6a5f\u6703\u5361\u4f4f\uff0c\u5176\u7a69\u5b9a\u5ea6\u8f03\u5dee\uff0c\u4e5f\u56e0\u70ba NobelReplace \u6700\u9577\u690d\u9ad4\u9577\u5ea6\u50c516mm\uff0c\u81e8\u5e8a\u4e0a\u8f03\u4e0d\u5be6\u7528\u3002\u81ea\u653b\u6027\u5f37\u7684\u690d\u9ad4\uff0c\u5982 \u8207\u4e00\u822c\u50be\u659c\u690d\u9ad4\u5b58\u6d3b\u7387\u76f8\u7576\uff0c\u4e5f\u8f03\u9874\u9aa8\u690d\u9ad4\u4fb5\u72af\u6027\u5c0f\uff0c\u6545\u53ef\u4ee5\u5c07\u5176\u61c9\u7528\u5728 All-on-4 treatment concept\uff0c\u540c\u6642\u4e5f\u7bc0\u7701\u6210\u672c\u53ca\u8cbb\u7528\u3002\u552f\u7368 technique
